Submit Reenrollment/Enrollment Form and required school documents by deadline. 6. BIS emails Distance Learning Guidelines, School Manual, Class Schedule, Academic Calendar, and Nurse's Note. Foreign citizens additionally submit authenticated Form 138, certified Form 137, Certificate of Good Moral, Forms 3 and 4, birth certificate with English translation, and passport/visa photocopies.
